What do we require to give you a Quote?
In order to conduct an accurate review, we require the following information.
- Signed letter of authorization (on your letterhead. Please fill in your policy number/s. Please note that this letter will NOT affect your relationship with your current advisor. It only permits us to obtain data, not take over your program)
- Completed employee data sheet Excel (78kb) or PDF (88kb) with all employees must be listed
- A copy of the employee booklet (if there is more than 1 class, we’ll need a copy for each class of employee)
- 3 years renewal history
- Sample billings for each of those 3 three years (preferably one month after each renewal)
- Copy of most current billing
- Status of any disabled employees
