What do we require to give you a Quote?

In order to conduct an accurate review, we require the following information.

1. Signed letter of authorization (on your letterhead. Please fill in your policy number/s. Please note that this letter will NOT affect your relationship with your current advisor. It only permits us to obtain data, not take over your program)
2. Completed employee data sheet Excel (78kb) or PDF (88kb) with all employees must be listed
3. A copy of the employee booklet (if there is more than 1 class, we’ll need a copy for each class of employee)
4. 3 years renewal history
5. Sample billings for each of those 3 three years (preferably one month after each renewal)
6. Copy of most current billing
7. Status of any disabled employees
